Last updated shows the publisher's own data date where the service reports one, and below it when this site last fetched from that source. Fetched data is reused for up to 30 days (searches: 7 days) before it is requested again.

The National Library of Medicine retired its drug-interaction lookup API in January 2024; interaction findings on this site are therefore drawn from the label text itself. ICD-10 prefix lookup table (122 rows)

A condition's ICD-10 code is matched to the longest prefix in this table — E87.5 wins over E87 — and the keywords of that row are what the check searches for in each drug's MED-RT contraindications and in the Contraindications and Warnings sections of its label.

Three-way combination rules (8 rules)

Some risks build up across three or more medications and are not visible in any single pair, so no one label describes them. Each medication's ingredients are sorted into the groups below — by their ATC drug class from RxClass, or by name — and a rule is met when the list has enough different ingredients in its groups. Every rule cites the source it rests on.

RuleNeedsGradeWhy, and the source
"Triple whammy" (kidney injury) ACE inhibitor / ARB / renin inhibitor + Diuretic + NSAID (not aspirin) Use caution Taking an ACE inhibitor or ARB, a diuretic and an NSAID together raises the risk of acute kidney injury, most in the first month after the third drug is added.
Lapi F, et al. Concurrent use of diuretics, angiotensin converting enzyme inhibitors, and angiotensin receptor blockers with non-steroidal anti-inflammatory drugs and risk of acute kidney injury. BMJ 2013;346:e8525.
Opioid with two or more sedatives Opioid + 2 × Sedative or other CNS depressant Use caution An opioid combined with other drugs that slow the brain (benzodiazepines, sleep medicines, gabapentin or pregabalin, muscle relaxants, sedating antihistamines) adds to the risk of dangerous sleepiness and slowed breathing; each additional sedative raises it further.
FDA Drug Safety Communications: opioids with benzodiazepines and other CNS depressants (Aug 31, 2016); gabapentin and pregabalin with CNS depressants (Dec 19, 2019).
Three or more drugs that raise bleeding risk 3 × Raises bleeding risk Use caution Anticoagulants, antiplatelets (including aspirin), NSAIDs and SSRI or SNRI antidepressants each raise the risk of bleeding, and the risk adds up with each one taken.
Each drug's FDA label (Drug Interactions / Warnings: concomitant use of drugs that affect hemostasis); de Abajo FJ. Drugs Aging 2011;28:861-875.
Three or more drugs that raise potassium 3 × Raises potassium Use caution ACE inhibitors and ARBs, potassium-sparing diuretics, potassium supplements and trimethoprim each raise blood potassium; together they can cause dangerous hyperkalemia. Potassium is usually checked with blood tests.
Each drug's FDA label (Warnings: hyperkalemia); Raebel MA. Hyperkalemia associated with use of angiotensin-converting enzyme inhibitors and angiotensin receptor blockers. Cardiovasc Ther 2012;30:e156-166.
Three or more serotonergic drugs 3 × Serotonergic Use caution Each serotonergic drug adds to the risk of serotonin syndrome (agitation, sweating, fast heart rate, tremor or muscle twitching, fever).
Boyer EW, Shannon M. The serotonin syndrome. N Engl J Med 2005;352:1112-1120; FDA Drug Safety Communication on opioids and serotonin syndrome (Mar 22, 2016).
Three or more drugs that prolong the QT interval 3 × Known risk of QT prolongation Use caution Drugs that prolong the QT interval add to each other's effect on heart rhythm, raising the risk of torsades de pointes, a dangerous arrhythmia; an ECG may be advised.
Drew BJ, et al. Prevention of torsade de pointes in hospital settings: AHA/ACCF scientific statement. Circulation 2010;121:1047-1060; CredibleMeds QTdrugs list (Known Risk of TdP).
Three or more strongly anticholinergic drugs 3 × Strongly anticholinergic Use caution Anticholinergic effects add up: confusion, memory problems, constipation, urinary retention and falls, especially in older adults.
American Geriatrics Society 2023 Updated AGS Beers Criteria. J Am Geriatr Soc 2023;71:2052-2081 (avoid combining anticholinergic drugs in older adults).
Three or more CNS-active drugs 3 × CNS-active (falls risk) Monitor For older adults, taking three or more drugs that act on the brain (antidepressants, antiepileptics, antipsychotics, benzodiazepines, sleep medicines, opioids, muscle relaxants) raises the risk of falls and fractures.
American Geriatrics Society 2023 Updated AGS Beers Criteria. J Am Geriatr Soc 2023;71:2052-2081 (any combination of three or more CNS-active drugs).
